She’d been working the triage desk for nine years, long enough to feel the slowdowns forming before anyone was waiting.

It wasn’t that sicker patients arrived. It wasn’t that the lab ran behind or the attending got slow. The slowdown had no center. It built itself from the spaces between things — from the narrowing gaps between one ambulance and the next, between one discharge summary and the next admit order. Each gap, on its own, was fine. Two minutes instead of three. Nobody noticed two minutes.

But somewhere in the corridor between intake and the treatment rooms, the spaces closed. And when the spaces closed, everything that had been moving freely began to press against everything else, and the system changed state.

Delia could feel it the way a sailor feels a change in swell. Not the wave itself — the wave hadn’t arrived. The period shortening. The intervals losing their cushion. She’d start prepping the waiting room chairs. Not because anyone would need them yet, but because someone would need them soon, and by the time the need was visible, the slowdown would already be forty minutes old and building.

The doctors never believed her. They’d look at the board — three rooms open, manageable census — and tell her she was borrowing trouble. Twenty minutes later, every room would be full and the waiting room would have twelve in it, and nobody could point to the moment it changed because the change wasn’t a moment. It was a gradient. A closing. The system didn’t break; it condensed.

What fascinated her, what she’d stopped trying to explain to anyone, was that the slowdown moved. Not forward, the way patients moved — left to right through the pipeline, intake to discharge. The slowdown moved backward. It started at the exit. Some bottleneck at discharge — a single patient waiting for a ride, one bed-assignment delay on the floor — and the pressure wave rolled back through the treatment rooms, back through triage, back to the waiting room, back to the ambulance bay. Every stage slowed, one after another, in reverse order. By the time the waiting room was full, the original bottleneck had cleared. The patient had gotten their ride. The bed had opened. But the wave was still traveling, and it would take another hour to dissipate.

She’d tried keeping records once. A notebook, then a spreadsheet. Arrival times, room assignments, discharge times, gaps between each. She thought if she could find the trigger, she could prevent the condensation. But the data showed her something she hadn’t expected: the slowdowns with identifiable causes were the small ones. A difficult intubation. A code. Those were local — they disrupted one room, and the pipeline flexed around them. The large slowdowns, the ones that turned a quiet Tuesday into a four-hour wait, had no trigger at all. They emerged from the ordinary rhythm of arrivals. Two minutes instead of three, repeated seventeen times, compounding.

The jam was made of nothing. Literally: it was made of the absence of gaps. And it was real in the way that a wave is real — it moved through the department with a shape and a speed and a duration, and every person it touched experienced it as waiting, as pressure, as the hallway getting harder — but no thing was the wave. The wave was a pattern of relationship between things.

Her daughter was in school for industrial engineering. Delia had tried to describe this to her once — the phantom jams, the backward propagation, the way the system had its own weather. Her daughter had gotten excited. “That’s queuing theory, Mom. That’s literally what I’m studying.” She pulled up diagrams, equations, Greek letters. Showed her simulations where dots moved along a line and bunched up for no reason.

Delia watched the simulation. “That’s it,” she said. “That’s exactly it.”

“So now you know why,” her daughter said.

Delia watched the dots bunch up, thin out, bunch again. The wave moving backward through the line of forward-moving dots. She’d been watching it for nine years in the bodies and voices of actual people. Her daughter had found it in the math.

“I knew why,” Delia said. “I just didn’t know you could see it without being there.”
